Instructions:

Choose the size of your ad (business card, 1/4 page,
1/2 page, or full page) & design your ad.
We PREFER you turn in your ad on a CD/disk (in *.jpg,
*.bmp, or *.tiff format.) Please make sure your ad is set to a 300
dpi resolution before submitting it.

If you would like us to design your ad for you, at
an extra charge of $5 + $1 for each additional person, provide us with your photos &
desired text, with any basic
ideas in mind.

Fill out & turn in the form below. If you are
turning in a personal ad as a group, designate ONE person in charge of
the group and fill out his/her information. Please list ALL the names of
those participating in the ad & each person's contribution($) to the ad
next to his/her name.

Turn in a check or cash for your ad. ONLY ONE
CHECK PER GROUP, please! Checks made payable to Kababayan. Program Ads
WILL NOT be accepted without payment, no exceptions.

TO AVOID LATE FEES, submit your ad & money by March
9, 2006 (9th week) @ the Kaba Meeting to either of
your Programs Coords, Jennifer Garcia or Stefanie Montemayor, or to one
of your PCN Coordinators, Jonathan Baucas, Kristin Eliazo, or Xavier
Hernandez.

Late Fees are as follows: Starting Wednesday March 15th, a $5
late fee will be imposed until the end of the quarter. Starting March
24th, the fee will have increased to $15 until the first Saturday
practice, April 8th, which is also the absolute last day to submit ads
and ad money.

If you are contributing money to
a suite ad, please submit your money to your suite coordinator. If
you are in multiple suites, you can contribute the appropriate amount of
money to each. For example, if you are in 3 suites, you can contribute $5
to each suite to satisfy the $15 requirement, unless one of your suites
requests a different amount of money.
Each PCN
participant is REQUIRED to buy or sell $15 worth of ads or
contribute $15 without ads.
